Move / Relocation Checklist

Things to Do
	If you are moving due to a life event such as a company job change/transfer, marital status change, termination or retirement, please review the appropriate checklists.	No Limit
	Change your name (including your preferred name), address or phone number.	No Limit
	Verify on the confirmation screen that the data you entered is correct. Due to technical limitations, your address can only be changed once per day.	No Limit
	If you move to a new state or into/out of a county with local taxes, you should complete a new state tax withholding election form. Until you file this form, your withholding election will automatically default to a filing status of single, claiming zero exemptions.	No Limit
	Change your emergency contact, if necessary.	No Limit
	If you move out of your medical or dental plan's service area, you may need to change your plan.	Within 30 days
	You may be eligible to make changes to your Dependent Care and/or Health Care Reimbursement Accounts and your Pretax Medical/Dental Premium Program.	Within 30 days
	If you have changed your checking or savings account, you may need to change the account where your payroll check is being directly deposited.	No Limit
	Call LifeWorks for tips on local school information or dependent care resources.	No Limit
	If you made any benefit changes that affect your paycheck, check your Earnings Statement to verify that the correct deduction is being taken out of your paycheck.	No Limit
	You also may want to update your records with the state department that issues drivers' licenses as well as with the Social Security Administration.	No Limit
	Your Group Universal Life coverage will not be affected unless you move into or out of the state of Texas, since Texas mandates different coverage levels from other states.	No Limit
