The YourCorp LIFE Insurance Plan automatically covers eligible employees at no cost.

Overview

Eligibility

You are eligible for coverage under the YourCorp LIFE Insurance Plan if you are an employee on the U.S. payroll and you are:

Coverage Begins

Coverage begins on an employee's first day of Active Work.

 

Coverage Ends

Your coverage ends if your employment is terminated, you are no longer eligible or in certain other circumstances.

Amount of Benefit

The amount of your YourCorp LIFE Insurance benefit under the Plan is $50,000.

Depending on the circumstances, you may also be eligible for an Accelerated Payment Benefit if you become terminally ill.

How and when benefits are paid

Benefits are paid within four to six weeks after all the information and documents needed to approve the claim have been submitted.

Death benefits and the Accelerated Payment Benefit are both paid in a lump sum.

  • Assign coverage?

To assign your coverage, you must complete an Absolute Assignment of Group Insurance form, which you can obtain from the Employee Services Organization.

  • Convert coverage?

Contact the Employee Services Organization before your coverage ends, and request the booklet explaining your conversion privilege.

How to Convert Coverage

  • Designate a Beneficiary?

Complete a Beneficiary Form and return it to the Employee Services Organization.

Beneficiary Information

  • Drop/Waive coverage?

Coverage in this free plan is automatic. It cannot be dropped or waived.

 

  • Enroll?

You are automatically enrolled if you are eligible.

 

  • File a claim?

Your beneficiary(ies) should call the Employee Services Organization within 90 days of your death to start a claim for benefits.

For the Accelerated Payment Benefit, you can initiate a claim by calling the Employee Services Organization.

Cost
