Before you retire |
|
 |
|
|
|
|
Confirm with your supervisor/manager what
your retirement date will be. Discuss with your
supervisor/manager if you want to extend your retirement
date by vouching Flexible Time Off. |

Before you retire |

|
|
|
Notify the Employee Services Organization
that you intend to retire and advise them of your
retirement date. Also advise them if you are eligible
for the International Retirement Guarantee or the Global
Retirement Supplement. A Retirement packet including
instructions and forms will be sent to you within 48
hours of your notification. |

Before you retire |

Complete your final timecard on a paper
timecard and send it (or fax it) immediately to the Employee
Services Organization. Contact your Administrative
Assistant for a blank paper timecard.
If you are an exempt employee, you should NOT
submit an Online Timecard for the pay period in which
you terminate. You must use a paper timecard for this
final pay period. |

When you retire |

If you would like to change your federal
or state W4 withholdings for your final paycheck, you
may review and elect to change your current
withholdings. |

Before you retire |

|
|
If you are taking a course for which
Educational Assistance is being provided, submit all
online Educational Assistance forms prior to retiring
and notify the YourCorp Education Card Administrator to
deactivate your card. |

Before you retire |

 |
|
|
|
|
Provide your supervisor/manager with any
Company equipment in your possession, such as name
badge/access card, computers (office/home), laptop,
printer, dictating equipment, answering machine, fax
machine (office/home), pager, cellular phone,
calculator, etc. |

When you retire |

|
|
|
Turn over all YourCorp confidential files,
account files, records, sample merchandise, display
material, etc., to the appropriate person in your
department. |

When you retire |

|
|
|
|
|
Notify the Fleet department and make
arrangements to return your Company car. Be sure to turn
in your Personal Use Declaration Form (provided by your
Fleet department), since this has an impact on the auto
income and taxes reported on your W2. |

When you retire |

|

|
If eligible, enroll yourself and your
spouse/domestic partner, if applicable, in the
Post-Employment Medical Plan within 30 days after
your retirement date or forever forfeit your right to
the coverage.
Note: If you have a
spouse/domestic partner who works for YourCorp and you are
considering obtaining health coverage through him/her at
your retirement, it is important that you contact the
Employee Services Organization to discuss the situation.

To convert your YourCorp LIFE Insurance,
request a copy of the conversion form. |

Within 31 days after retirement |

spouse/domestic partner is an active YourCorp employee at the
time of your retirement)
For dental coverage, you
and the dependents you cover may be enrolled in your
spouse/domestic partner's coverage. To enroll, your
spouse/domestic partner must follow the enrollment
process. |

Within 30 days after retirement |

You may be eligible to continue your
medical and/or dental coverage through COBRA after you
terminate. You must request a copy of the COBRA Election
form, complete it, and return it to the COBRA
Administrator within the required time. |

If applicable, update your address/phone
to ensure you continue to receive important mail, such
as your W-2. If you're planning to move after your
retirement date, use the hardcopy Change of Record Form
in your retirement packet, and return (the form) to the
Employee Services Organization. |

You also may want to update your records
with the state department that issues drivers' licenses.

Once you retire, you may no longer
contribute to any Reimbursement Accounts. However, you
may submit claims for eligible Dependent Care and Health
Care expenses incurred through December 31st of the year
in which your contributions end. |

If you are eligible for the Retiree
Survivors' Benefit Plan, identify your primary
beneficiary by completing the Retiree Survivors' Benefit
Plan Designation form that you receive in your
Retirement packet. |

If you wish to continue your Long-term
Care coverage, notify the Program Administrator to
arrange for direct billing. If you do not participate,
you and your other eligible family members may enroll. |

Universal Life coverage, notify the Program
Administrator to arrange for direct billing. |

Soon after retirement |

The Employee Services Organization will
send you a letter advising you of the taxes that you owe
for the vesting of your Restricted Company Shares in the
Stock Purchase Plan. Pay the taxes immediately to avoid
a delay in receiving your shares. |

6 to 8 weeks after retirement |

|
You must exercise any unexpired qualified
stock options. |

Within 3 months after retirement |
 |

|
You must exercise any unexpired
non-qualified and discounted stock options. |

Within 3 years after retirement |

If you are eligible for the Gold-colored
Badge, two badges will be mailed to your home address
shortly after your retirement. If you do not receive
these badges, contact the Employee Services Organization
for replacements. |
